According to Nielsen Korea, the fourth episode recorded an average nationwide rating of 7.5 percent.
'Thirty-Nine' Episode 4 Recap: Jung Chan Young Resist to Get Treated
Titled "Choice", the fourth episode showed Jang Chan Young's (Jeon Mi Do) shock after learning how bad her health is after learning about the heartbreaking news.
She was told that there's only a 0.8 percent survival rate if she resists chemotherapy and one year if she decides to undergo it.
Clinging to the slim chance, Mi Jo comforts her friend, saying there's still hope, but Jang Chan Young seems like she has made up her mind and will not undergo chemotherapy.
This made Mi Jo disappointed and somehow furious that her friend was already giving up; however, the 39-year-old acting teacher explained that she wanted to live her life normally and not spend her remaining days at the hospital lying in bed.
Unaware of the situation, Jang Joo Hee (Kim Ji Hyun) called the duo and told her about the lottery.

Hearing how thrilled she was, they decided not to tell Joo Jee about Chan Young's health condition.
Jang Joo Hee's Shock
Wanting to celebrate her small win, Jang Joo Hee waited for her friends at the restaurant and ordered the most expensive meal.
Little did she know that Jang Chan Young and Cha Mi Jo were looking at her outside. The acting teacher didn't want to inform her about her illness, saying "look how happy she looks," but Mi Jo doesn't know if she could hide it from Joo Hee.
While having dinner, Jang Joo Hee noticed that Jang Chan Young was not drinking and Mi Jo was not herself lately.
Sensing that the two are hiding something from her, she gets honest and tells them that she always was the last one to know.
Finally, Chan Young got the courage to reveal it to Joo Hee. Initially, she mentioned that her mother had cancer too but was now cured; however, Mi Jo explained that Chan Young has stage 4 cancer and has a slim chance of survival.
Of course, this made her devastated and she cried all day long at her work.

"Thirty-Nine" episode 4 featured the sacrifices of Cha Mi Jo and Jang Joo Hee for their beloved friend.
The trio gathered at Jang Chan Young's house with Mi Jo surprising them with luxury goods.
Here, the head dermatologist vowed to look at the brighter side and spend the remaining days making their friends happy.
She also tells them that she will still have her sabbatical but will dedicate it to looking after her friend.
The same goes for Jang Jo Hee, who gave her luck by shredding the lottery ticket.
"I've decided to give that luck to Chan Young," she said.
