"Modern Family" star Sarah Hyland recently filed for a restraining order against her ex-boyfriend of five years, Matt Prokop.

The 23-year-old actress revealed that Prokop physically and verbally abused her while they were still in a relationship and insisted that he even choked her once.

Hyland's attorney, Lee A. Sherman, released an official statement and said, "The documents filed speak for themselves. Out of respect for the court, the process and all parties, I have advised Ms. Hyland not to comment on the matter. We request that you respect the parties' privacy during this time."

In the court documents initially obtained by TMZ, Hyland narrated several instances wherein she was physically abused and threatened by her 24-year-old ex-boyfriend.

At one point, Hyland also shared an instance wherein during an argument, Prokop pinned her against a car and choked her and called her a c*unt over and over again, the Daily Mail reported.

"His grip was so tight that I could not breathe or speak. I was scared an in fear for my life," she wrote in the court filings.

After five years of domestic violence, Hyland then reached out to her "Modern Family" co-star Julie Bowen, who helped her end the relationship for good, the same publication noted.

After ending the relationship, Prokop continued to send Hyland threatening and disturbing text messages and voice mails including suicide threats, according to Contact Music.

Back in August, multiple sources claimed that Hyland and Prokop have already ended their relationship. A source then told Us Weekly, "She's young. They decided they would be better off as friends."

Prokop eventually entered into a rehab and when he was sent out, someone from the rehabilitation center contacted Hyland and advised her to file for a restraining order to ensure her safety.

The restraining order has been granted in Los Angeles and it requires Prokop to stay at least 300 feet away from her actress and her dog.
